What this HOMA-IR calculation measures

HOMA1-IR estimates insulin resistance from paired fasting glucose and insulin. It is a model-based index. A single result does not diagnose insulin resistance or diabetes.

Use paired fasting results

Enter glucose and insulin measured from the same fasting blood draw. Insulin should be in µIU/mL or mIU/L; those two unit labels have the same numerical value.

Choose the glucose unit

The HOMA1 approximation uses one of these equivalent conventional formulas. Use the unit printed on your glucose result.

mg/dL × insulin ÷ 405 or mmol/L × insulin ÷ 22.5

Keep the testing context

Assay methods and the population being assessed affect interpretation. Oxford does not define a universal normal/abnormal cutoff. Review results with a clinician who knows the testing conditions.

Example: glucose 90 mg/dL and insulin 5 µIU/mL

90 × 5 ÷ 405 gives a HOMA1-IR estimate of 1.11. The result describes the entered example; it does not place someone in a diagnostic category.

Common questions

What is the HOMA-IR formula?

For the HOMA1 approximation, multiply fasting insulin in µIU/mL by fasting glucose in mmol/L, then divide by 22.5. With glucose in mg/dL, divide the product by 405 instead.

Is this the HOMA2 calculator?

No. This page uses the HOMA1 linear approximation. Oxford’s HOMA2 calculator is a separate computer model and can produce different results.

What is a normal HOMA-IR result?

There is no universal cutoff. Oxford notes that HOMA indices depend on the specific laboratory assays. The calculator therefore reports the index without assigning a normal, high, or diagnostic label.

Can I use a non-fasting result?

This model expects a fasting steady state. A recent meal changes the glucose–insulin relationship. Use paired fasting results and check suitability with the clinician who ordered the tests.

Can I use insulin reported in pmol/L?

The form expects µIU/mL or mIU/L. Ask your laboratory for the appropriate assay-specific conversion if it reports pmol/L.
