Sterile Technique Checklist for Peptide Reconstitution

Aseptic technique at home comes down to a short sequence performed the same way every time: clear and clean the work surface, wash your hands, lay out supplies without touching what will contact solution, swab both stoppers and let them dry, use a new needle for every entry, and dispose of sharps immediately. The order matters as much as the steps.
Be clear on the ceiling, too. Compounding standards for sterile preparations assume engineered air handling and trained staff. Careful home practice lowers risk; it does not match a pharmacy.
Before you start: the setup
Set up before you open anything. Once a stopper is swabbed you should not be walking across the room for supplies.
- Clear a hard, non-porous surface. Kitchen counters and glass tables work; fabric, wood cutting boards, and bathroom counters next to a sink do not.
- Wipe the surface with 70% isopropyl alcohol and let it dry fully.
- Close windows and turn off fans, air purifiers, and HVAC vents blowing directly on the area. Moving air moves particles.
- Remove pets from the room, tie back long hair, and take off rings and bracelets.
- Wash hands with soap and water for at least 20 seconds, or use an alcohol-based hand rub, following standard hand hygiene guidance. Dry thoroughly.
- Lay out supplies: the compound vial, diluent vial, syringe, needle or needles, several alcohol prep pads, a vial label, and an open sharps container.
Anything you might need mid-procedure should already be within reach. The most common break in technique is a mid-procedure trip to a drawer.
The reconstitution sequence
Run the same sequence every time so the order becomes automatic rather than remembered.
- Inspect both vials. Check the compound label, the diluent label, expiration dates, and the solution for particles or discoloration. Confirm the diluent is what you intend to use.
- Remove the plastic flip caps. The rubber stopper underneath is not sterile.
- Swab the compound vial stopper with a fresh alcohol pad, using firm strokes in one direction, and let it air dry completely.
- Swab the diluent stopper with a second fresh pad. Let it dry.
- Attach a new needle to the syringe. Do not touch the needle, the hub, or the syringe tip.
- Draw your planned volume of diluent, keeping the needle inside the vial while you adjust the plunger and expel air.
- Insert the needle into the compound vial at a slight angle and direct the diluent down the inside wall of the vial rather than onto the powder directly.
- Withdraw the needle and discard the whole needle and syringe assembly in the sharps container. Do not recap by hand.
- Swirl the vial gently, or roll it between your palms, until the powder dissolves. Do not shake.
- Label the vial immediately with compound, concentration, and date and time of first puncture.
Foaming is the sign you were too vigorous. Peptides are physically fragile in solution, and mechanical agitation is one stress they do not tolerate well.
The per-dose sequence
Every subsequent draw repeats a shorter version. Treat each entry as a new procedure rather than a continuation of the last one.
- Wash or sanitize hands.
- Inspect the solution for cloudiness, particles, or color change. Stop if anything looks different.
- Swab the vial stopper with a fresh alcohol pad and let it dry, every time, even if the vial has only been sitting for a day.
- Use a new needle and syringe. Draw your calculated volume, expel air, and verify the volume against your concentration before you set anything down.
- Swab the injection site and let it dry.
- Inject, then discard the needle and syringe in the sharps container without recapping.
- Log the dose, site, vial, and time.
What technique controls, and what it does not
Technique manages contamination introduced during handling. It has no effect on what was in the vial when it arrived, and being clear about that boundary prevents false confidence.

That last row is the honest limit. Standards for sterile compounding exist because visual inspection and good intentions do not demonstrate sterility.
Mistakes that recur
Most technique failures are small shortcuts taken under time pressure rather than gaps in understanding.
- Puncturing a wet stopper, which negates the swab.
- Reusing a needle for a second entry to save a step.
- Swabbing only the compound vial and treating the diluent stopper as clean.
- Touching the stopper face after swabbing to check whether it is dry.
- Recapping needles by hand, which is the classic needlestick scenario.
- Working next to a running sink or an open window, both of which add aerosols and particles.
- Skipping the label, which turns a well-executed procedure into an unidentifiable vial three weeks later.
When to stop and discard
Some observations end the session. Cloudiness, visible particles, a color change, a crack in the vial, a stopper that appears cored or damaged, or a vial past its beyond-use date are all reasons to stop rather than reasons to look more closely.
The same applies to procedural breaks: if a needle touches a non-sterile surface, if you are unsure whether a stopper was swabbed, or if a vial has been sitting out uncapped, treat the item as compromised. Discard sharps in a proper container, follow local disposal programs for pharmaceutical waste, and record what happened in your log so your inventory math stays accurate.
